Obtaining a letter of Disability
To apply for a letter of disability:
A. You may request assistance from the local Veterans Affairs office
OR
B. You can send:
- Completed & signed Department of Veterans Affairs VA form 21-4138/Statement in support of claim (printable from below) [Fill in your name, Social Security No., Address, Telephone Numbers, Date, and Sign]
- A copy of the sample disability letters from Veterans Affairs (printable from below) to:
