There are two steps that must be completed for you to continue in our hiring process:
1. Authorization to Release Information Document
This Authorization to Release Information document must be notarized. Print out the form and complete it in the presence of a legal notary

2. Driving Record Document
Click on the state that your current driver's license was issued by to obtain the required documentation for a check of your driving history.
* Virginia Drivers - print out your driving record form and submit it to the Human Resources Department who will run the driver check free of charge.
Once you have filled out all required documents, you must submit them to the Human Resources Department. Documents may be scanned and uploaded via the "Attachments" tab of the NEOGOV application, scanned and sent via email to hr@fcva.us, mailed to 107 North Kent Street c/o HR or delivered in person to Suite 2200.
